The KCRC blamed staff negligence for causing a 60-metre girder to collapse on a West Rail work site in Kam Tin on November 28, resulting in the temporary closure of a section of Kam Tin Road. It said the contractor's supervisor failed to follow the established procedures and checklists. Extra precautions have been put in place following a review of the accident.
