Count instruments, surgical staff told

Queen Elizabeth Hospital apologised again to a 72-year-old patient for leaving a 20cm copper plate inside her body after an operation to remove a tumour from her pancreas last month. Doctors had operated again to remove the instrument, a copper retractor. A Hospital Authority investigation panel recommended that operation theatre staff ensure instruments are accurately counted before surgical wounds are closed, the hospital said yesterday.
