The root of the problem is in corruption which had caused widespread moral decay; more specifically with medical services it is partly caused by low wages of doctors so that many had to supplement their income from bonuses from tests and treatments, and kickbacks from prescriptions. As for food safety, problems are numerous: it is the whole system as many violators are SOE and enforcement by local agencies are not properly staffed and supervised - qualifications being party membership and loyalty.
