Chief Executive Tung Chee-hwa has failed to win Legco's stamp of approval on his policy address for the fifth time.
After three days of debate, pro-democracy lawmakers successfully blocked the traditional motion of thanks for the policy blueprint.
But the amendment seeking to embarrass Mr Tung by turning the motion into one expressing regret was rejected.
This is the third time in a row that Legco has refused to 'thank' Mr Tung for his policy speech. Members also voted twice to reject his address during his first term.
Wrapping up the debate, Legco House Committee chairwoman Miriam Lau Kin-yee urged Mr Tung to hold question time with lawmakers more often.
'Mr Tung should take the lead to improve relations with the legislature,' she said, referring to the cap of only four sessions with Mr Tung a year.
Speaking earlier in the debate, banking representative David Li Kwok-po said air pollution was a threat to Hong Kong's standing.
